142d Force Support Squadron
Customers must have two forms of ID, one of which must be a valid photo ID.
If CAC is lost or stolen, you must fill out AF Form 1168 with Security Forces prior to your scheduled appointment.
Hair and facial hair must be in Military Regs.
Our Booking PolicyPlease book ONE person per 20 minute appt slot. All individuals MUST bring two forms of ID, including a valid photo ID. Sponsors must be present to sign a DD Form 1172 that is generated in our office during the appt.
